SUBJECT: Pertussis, Meningococcal and HPV Vaccination for 7th Graders
Virginia Code § 22.1-271.2. requires all students who are entering 7th grade to receive a booster dose of a pertussis-containing vaccine, the meningococcal conjugate (MenACWY) vaccine, and the Human Papilloma Virus Vaccine (HPV). However, after reviewing educational materials approved by the Board of Health, the parent or guardian, at the parent's or guardian's sole discretion, may elect for the child not to receive the HPV vaccine. If your child has already received his/her vaccinations, please send a copy of his/her vaccination record to school.
The Three Rivers Health District will provide Tdap, HPV, and Meningococcal Conjugate (MenACWY) vaccines during the upcoming vaccine clinics to be held on Monday, May 6, 2024.

If your child has private insurance/FAMIS/Medicaid, please enter the coverage information on the consent form as requested along with a copy of the front and back of your insurance card. Please return a completed vaccination consent form to school by Friday, May 3, 2024, to ensure adequate vaccine supply is ordered.
